○参加申し込みは専用エントリーフォームにてのみの受付となります。 　（レーティングフォームの送信ではエントリーとなりません。）

○変更、キャンセルはお申し込みになった店舗へご連絡ください。 　※変更・キャンセル締切日以降のキャンセルについては、 エントリーフィーは ご返却できませんので、 　ご了承下さい。

○当日キャンセルの場合、エントリーフィーは返却されませんので、ご了承下さい。

○エントリーフィーのご入金が確認でき次第、エントリーの完了となります。 　※締切日までにご入金が無い場合は、キャンセルとさせていただきます。

○DJOオフィシャルショップ以外の店舗様からのエントリーは、SHOP登録費として1大会につき、 　3,000円とさせていただいております。エントリーフィーと一緒にお振込みお願い致します。

○ディーラー登録費年間20,000円 が必要となります。ディーラー様にご確認お願い致します。

○Please use the “DJO Entry Form“to participate in each tournament. (Please make sure to send the “ENTRY FORM” and NOT the “Rating Form”.)

○Please contact the shop where you made your application for changes in entry and cancellation. Please note that fees would not be returned to any cancellation request past the deadline. Thus we do accept cancellation on the day of the tournament, but there will be no refund.

○Entry is fully complete after the fee is wire transferred to the assigned account. If no transaction is seen until the entry deadline, it will be considered canceled.

○All shops except DJO official shops are required to pay a registration fee of \ 3,000 with the entry fee for each tournament.

○All the dealers participating in the tournament are required to pay an annual fee of \ 20,000.
